Name Change Forms
These are the name change forms that are available, free of charge, at the Family Law Self-Help Center.
You must have Adobe Reader (or above) to open and use the forms. "Pdf fillable" forms can be saved to your computer and then you can type in them. "Pdf" forms cannot be completed on your computer, but you can print and complete by hand.

AUTOMATED FORMS INTERVIEW AVAILABLE!
There is an automated interview available that will complete your forms for you after answering some questions. To use the automated interview, please click here and select the "NAME CHANGE: Adult Name Change" interview. It is best to use Chrome or Firefox (Safari is not recommended and not supported).
At the end of the interview you will have to save your forms and then file them with the family court.
Some of these forms are needed if both parents are agreeing to the name change, and some are needed only if one parent is requesting the name change. Visit Child Name Changes to make sure you are using the correct forms depending on whether both parents are in agreement.
