Custody Paternity & Child Support Forms
This is a list of the custody and paternity forms that are available, free of charge, at the Family Law Self-Help Center.

Use the "Custody" forms if the father is known and there is no disagreement about paternity. Use the "Paternity" forms if you mainly need the court to determine who is (or who is not) the father.
